Extension School degree and certificate candidates who are registered for a minimum of 12 units per term are eligible for health insurance through the Harvard University Student Health Insurance Plan. Spouses and/or children also are eligible for coverage. For further information, candidates should call Student Financial Services at (617) 495-9360. A $423.68 health insurance fee will be charged each term to degree and certificate candidates who are admitted to the program by the end of the registration period and who are registered for a minimum of 12 units. The fee will be charged automatically. To waive the fee students must submit completed and signed insurance waiver forms either at the time of registration or by October 8 for fall and February 18 for spring to avoid the insurance charge. It is extremely important that waiver forms be submitted by the deadlines as insurance charges cannot be reversed or prorated after the deadlines have passed. Waiver forms will be mailed to candidates; they also are available at 51 Brattle Street. Candidates registered in fall courses who indicate on their waiver forms that they wish to waive coverage for both fall and spring terms need not submit separate waiver forms for the spring term. Students who are not degree or certificate candidates, as well as candidates enrolled in fewer than 12 units, are not eligible for insurance coverage. |
